Payments

There are several ways to make a payment.

Paypal offers a variety of options. It allows payments from credit cards, debit cards and your bank account. A PayPal account is not required. To submit payment via PayPalPlease please enter the information from your invoice. Please do not include personal health information. This form is for payment purposes only.

You may also pay via Venmo. My Venmo name is Douglas-Cohen-10. If Venmo asks for the last four digits of my phone number, the number they want is 2852. You do not need to ask me to friend you and there are some problems if you do that.
